Insight Renewal Center: Website and Patient Privacy Policy

billing for services, and practice management.

• Exceptions to confidentiality

We may disclose PHI without your authorization in specific situations required or

permitted by law. These include mandated reporting for abuse or neglect, when

there is a serious threat of harm to yourself or others, in response to court orders

or other legal processes, and to comply with other legal requirements.


5. Your rights regarding your PHI

Under HIPAA, you have several rights concerning your PHI, including the right to access and copy your records, request amendments, receive an accounting of disclosures, request restrictions on use and disclosure, and request confidential communications. You also have the right to receive a paper copy of this policy.

We use secure, HIPAA-compliant platforms for telehealth services. Please ensure your location is private during sessions.

For questions or to file a complaint about privacy concerns, you may contact us or the U.S. Department of Health and Human Services. Filing a complaint will not result in retaliation.
